Patient Agreement

Bison Drugs Canada is a duly licensed pharmacy by the College of Pharmacists of Manitoba (license 36010) in Winnipeg, Manitoba, Canada. All products are Health Canada approved and purchased from Canadian wholesalers. You agree to appoint Bison Drugs Canada as your agent with respect to facilitating the delivery of your pharmaceutical product(s); giving the authority to the agent to sign on any documents on your behalf as necessary to obtain a valid prescription in Canada where the prescription shall be filled. The prescription written in Canada shall be equivalent to the product prescribed by the original prescriber. This authorization extends to allowing Bison Drugs Canada to collect personal health and information data for the sole purpose of providing you with healthcare services. It shall not be disseminated to other parties (aside from healthcare practitioners who are bound by the Personal Health Information Act) without your written consent. You agree to allow Bison Drugs Canada to repackage as necessary the pharmaceutical products for the purpose of delivering to you. Any prescriptions you send to Bison Drugs Canada must have been written from your primary care provider and lawfully obtained. Furthermore, the product will be used as prescribed according to the instructions written by the primary care provider.
